CHA2-DS2-VASc Score calculator is a medical tool used to calculate the risk of having cerebrovascular accident (CVA) also know as Stroke in patients that have Atrial Fibrillations.
This calculator also gives recommendation of whether oral anticoagulants (blood thinners) like warfarin are indicated or not based on your score.
It takes into account having comorbidities like Hypertension, Diabetes mellitus, having history of TIA and Heart Failure among others.

If the score is less or equal to 2, some guidelines recommend aspirin use while others state that it is not necessary to use anticoagulants.
Patients that qualify for warfarin long term use, i.e. those with score of 2 or more are given warfarin as follows:
Warfarin 5mg oral daily
INR is checked after 48hours and repeated every 1 to 2 days until it is within the therapeutic range of 2 to 3.
Warfarin dose is adjusted to keep INR within target dose range.
